If you know anything about dental disease, then you know bacteria ranks high on the Usual Suspects list. Tooth decay gets its start from acid produced by bacteria; periodontal (gum) disease is often triggered by bacteria that infect the gums.
But the particular strains of bacteria that can cause dental disease are a small percentage of the 10,000-plus species inhabiting your mouth. The rest, numbering in the millions, are fairly benign—and some, as recent research is now showing, play a sizeable role in protecting your teeth and gums against other malicious bacteria, fungi and viruses.
Dr. Aaron Weinberg, a dental researcher at Case Western Reserve University, has been investigating these protective bacteria for many years. His research began with a scientific conundrum: although the mouth has one of the highest densities of bacterial populations, wounds in the mouth tend to heal quickly.
The answer, he believes, originates with human beta defensins (hBDs), substances produced by cells in the lining of the mouth that are natural antibiotics against disease. He has found that certain bacteria actually help stimulate their production.
This isn't just an interesting fact about the body's defenses and immune system. During his research, Dr. Weinberg was able to identify the agent within the bacteria that triggered hBD production. This has opened up a new line of research: The possibility that harnessing this agent might help assist in our treatment of infection by boosting the body's defensive capabilities.
For example, researchers have proposed including a form of the agent in toothpaste. Over time, this might stimulate hBD production and guard the mouth against the development of dental diseases like gum disease.
These possibilities all come from our increasing knowledge and understanding of the microscopic world around us, especially in our mouths. Bacteria are much more complex than we may have realized—not all are our enemies, and some are definitely our friends. Learning more may open up new ways to keep our teeth and gums healthy.
After months of wearing braces it's time for the big reveal: your new and improved smile! Your once crooked teeth are now straight and uniform.
But a look in the mirror at your straighter teeth might still reveal something out of place: small chalky-white spots dotting the enamel. These are most likely white spot lesions (WSLs), points on the enamel that have incurred mineral loss. It happens because mouth acid shielded by your braces contacted the teeth at those points for too long.
Most mouth acid is the waste product of bacteria that thrive in dental plaque, a thin film of food particles that can build up on tooth surfaces. High levels of acid are a definite sign that plaque hasn't been removed effectively through brushing and flossing.
But normal hygiene can be difficult while wearing braces: it's not easy to maneuver around brackets and wires to reach every area of tooth surface. Specialized tooth brushes can help, as well as floss threaders that help maneuver floss more easily through the wires. A water irrigator that uses pulsating water to remove plaque between teeth is another option.
However, if in spite of stepped-up hygiene efforts WSLs still develop, we can treat them when we've removed your braces. One way is to help re-mineralize the affected tooth surfaces through over-the-counter or prescription fluoride pastes or gels. It's also possible re-mineralization will occur naturally without external help.
While your teeth are sound, their appearance might be diminished by WSLs. We can improve this by injecting a liquid tooth-colored resin below the enamel surface. After hardening with a curing light, the spot will appear less opaque and more like a normal translucent tooth surface. In extreme cases we may need to consider porcelain veneers to cosmetically improve the tooth appearance.
In the meantime while wearing braces, practice thorough dental hygiene and keep up your regular cleaning visits with your general dentist. If you do notice any unusual white spots around your braces, be sure to see your dentist or orthodontist as soon as possible.

With the advent of home whitening kits, you no longer have to go to the dentist to have your teeth whitened. DIY kits are relatively safe and effective, if you follow the directions carefully.
So, you might be thinking: why have a dentist whiten my teeth? Actually, there are good reasons why you might. Here are 4 of them.
We'll make sure your teeth are healthy first. Your teeth may need some attention first, such as treatment for dental disease, before we undertake whitening. We'll also determine why your teeth are stained, which could impact how they're whitened (more about that in a moment).
Our application could take less time and last longer. Bleaching agents in home kits make up less than 10% of volume, much weaker than the applications we use. While it often takes several applications at home to achieve the desired brightness, you may only need one or two sessions with us. Our stronger solution may also extend the “fade time” — when the whitening begins to diminish — than what you may encounter with home whitening.
We can be more precise achieving the right shade. There are different shades of teeth whiteness — what looks good for someone else might not look good for you. We have the training and expertise to achieve a color that's right for you. What's more, we also have techniques and equipment like UV lighting that enables us to color match more precisely than you can with a home kit.
Your DIY kit can't alter some forms of staining. Home kits bleach only the outermost layers of tooth enamel. That won't help, though, if your discoloration originates inside the tooth. This intrinsic staining requires procedures only a dentist can perform to bleach the tooth from the inside out.
Even if you'd still like to use a home kit we'll be happy to advise you on purchasing and application. It's also a good idea to have us check the staining first to see if a home kit will work at all. In the end, we share the same desire as you do: that your teeth are as healthy as they can be and bright as you want them to be.
